Forms & documents

These forms are for submitting paper claims, applying for disability benefits, changing your banking information, updating an address, adding/changing a dependent, and assigning a beneficiary for your life insurance, among others.

You can submit your forms via email or fax, but they must have a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.

Accessing your benefits

As a member of the Electrical Industry of Ottawa Group Benefit and Pension Plans, you will be able to access your benefits by submitting claims for medical expenses incurred.

You will be able to submit health claims electronically by logging into the Ellement App; however, some providers have the option to direct bill.
